After changing the theme, the options are not showing.

Enable the app in your Shopify theme settings and save the changes. 

You can do this from the app dashboard by following Setup Guide>>Step 1: Enable App Embed 


Or go to Sales Channels>>Online Store>>Themes>>Edit theme>>App embeds, enable the app, and save.
